Just when you think you are getting somewhere, there’s more paperwork to do, redo and clarify. Here’s the latest from Lifeline on our home study progress. On a good day, this is a lot of phone calls, waiting, driving to pick up and emailing/scanning of documents.
“Hi Beth,
It was good to talk with you yesterday. As we discussed, my supervisor has finished her file review, and there are a few things we need clarification and/or additional information concerning:
1) Your Pediatric Reference is missing (I’ve enclosed a blank form for ease of reference; Note that the address on the form is our old one, so the doctor would need to send it to 2104 Rocky Ridge Rd, Birmingham, AL 35216 or email/fax it. 2) An MD Letter for Dale regarding his medication (This should cover what medication he is taking, what it is for, duration/frequency of usage, prognosis, and his suitability to adopt statement); Also, his overall MD letter discussing his CA diagnosis and treatment will expire in July. Your primary agency will need a new one, and it needs to explain diagnosis, treatment, prognosis, and suitability for adoption.
3) Your MD also left off the result of your TB test (has a date, but no result listed). If you have the lab report for this, you can send that in to suffice. We have to be able to put that a negative result was noted.
4) Lee’s Medical indicates that she is on Buspar and other medications: We will need an MD letter explaining her medications and with a recommendation about her suitability to be around children and residing in a home with adopted children, etc.
5) Additional reference from your Accountability Group (If you provide the contact info, I will send the reference)
6) Digital Photos of Home (Interior and Exterior) and Family
Also, can you confirm for me that you are not on any medications? And if not, how long you haven’t taken anything? Your initial application on file (from June 2010) had indicated medication for your arthritis/pain.

We have the final home study interview today. It feels good to be at this point in the process. We’re ready to get our girl!
